Service Dog Application Process

Step 1: Preliminary Application

Apply online now by downloading our Service Dog Preliminary Application. There is no fee for this application.

Find out if a service dog will enhance your quality of living. You must have a diagnosed disability and be currently enrolled in at least one therapy or therapeutic program.

Step 2: Service Dog Application

Once the Preliminary Application has been approved, a ​Full Service Dog Application will be mailed to you. This application is designed to help us get to know you better and understand your needs more thoroughly. A referral from a Service Provider and Doctor are required as part of the application. There is no fee for this application.

Step 3: Telephone Conference

A telephone conference will be scheduled to discuss your application responses in further detail. Your referring Service Provider and Doctor will be contacted to gain a full understanding of how they believe a service dog will enhance your quality of living. 

Step 4: Meet Your Selected Service Dog

A service dog will be selected for you based on your needs. A meeting will be planned so that you can get to know the service dog selected specifically for you. You must pay your travel costs.

Step 4: Service Dog Training Agreement

After meeting your selected service dog, a Service Dog Training Agreement will be provided. This agreement sets forth the training for the service dog selected specifically for you. All of the details about the week-long home delivery, health guarantee, and follow up training for life are provided. The Service Dog Training Agreement must be returned with 50% down payment of the total service dog cost. Once received, the service dog training will begin and lasts 12-24 months, on average, to complete. Your service dog will be fully trained for you during this time. 

Step 5: Delivery

Once your service dog is fully trained, he or she will be delivered to you. Delivery is conducted in your home from morning until evening over the course of 5 days. You must be home and schedule all appointments or activities you regularly attend such as therapy sessions, grocery shopping, dining out, and religious services. Costs you must pay include the 50% final payment, our travel to your home, lodging, and meals.

You and your service dog are supported with free training with us for life and your service dog's health is guaranteed! We are committed to the lifelong partnership between you and your service dog. Refresher training is wonderful when you are already planning to board your dog. You cover the boarding fees at our boarding provider and we provide the training at no additional cost.
